Fill out our "REQUEST FOR I-20 or DS-2019" form (this form can be found at http://... or at our office) and make an appointment to meet with one of our advisors. Please also bring your passport and proof of health insurance to the office for verification. The process to re-issue a new I-20 normally takes (NUMBER OF DAYS). However, it may take longer depending on the specific time of the year.
