1. Complete the Dependent Request form and the Dependent Biodata form to invite your dependents (Spouse or children only)



2. Show proof of financial documentation to fulfill financial requirements. Your financial document must show the funds available to support your spouse at $375/month in addition to health insurance of approximately $1000/year.
   
   or $5,500 per year and $200 per month plus health insurance of approximately $1000/year or $3,400 per year for each child. You must also assume financial responsibility for the purchase of health insurance and the return travel of your family to your home country. Your financial documentation may be a bank statement, assistantship letter or a combination of both.



3. Health Insurance.
   
   F-1 Visa holder: It is Syracuse University policy that all international students and their dependents be covered by health insurance. Your family must obtain health insurance prior to or within 30 days after arrival.


If you have a graduate assistantship and have purchased health insurance through the Office of Human Resources, you must extend your coverage to include your dependents as well as purchase additional supplemental medical evacuation and repatriation coverage through the Slutzker Center.

J-1 Visa holder: Department of State regulations require that J-1 Exchange visitors (and all J-2 dependents) must be covered by an insurance policy which provides adequate coverage for medical expenses incurred while they are in the US.
