Before your arrival in the U.S.

  1. Fill out the MEDEX application form. Make sure to sign it.
  2. Send the application form and your payment to MEDEX at the address indicated on the form. If you are using your credit card, you may scan your complete application form and e-mail it to dwinslow@medexassist.com.
  3. You will receive a MEDEX card in mail.
  4. Please follow the above instructions to renew your MEDEX coverage before it expires.
